In my case, I started out in practice in button down oxford shirts and workpants - and couldn't get men to look me in the eye when I talked to them. Dress in layered t-shirt, scrubs and smock - and they ignored the large squishy things on my front and paid attention to the words coming out of my mouth. I think it all depends on your situation.
