My solution to this, so far, has been to exhaust myself to the point where I would likely be able to fall asleep even in the gaping maws of hell itself, and while it's less than optimal it at least seems to be keeping me functional.
Another idea to keep in mind for the future is to get in the middle of the bed and put pillows on both sides of you, like a big nest.
