Italy has very different regions, almost like different countries. The train is good between cities, for the smaller historic towns driving is OK - but get a guide on the rule differences first. You will find probably find Italian roads far too fast and small. In cities - forget cars.
Get a guide like "lonely planet". Prices for food vary hugely, and away from touristy places are fine.
We had no real issue not speaking Italian around tourist attractions, and elsewhere was fine.
